1. What this portal is. The H.E.A.L. Network patient portal lets you complete your questionnaire, connect a wearable device, share documents, and communicate with our care team. Access is by invitation only.
2. Not emergency or diagnostic care. The information and coaching provided through this portal is for functional-health and wellness support. It is not a substitute for emergency care or for advice from your own licensed medical provider. If you have a medical emergency, call your local emergency number.
3. Your account. Keep your password private and do not share your login. Tell us right away if you think someone else has accessed your account.
4. Wearable data. If you connect a device such as your Oura ring, you authorize us to retrieve and store your data from that device to support your care. You can disconnect at any time from your dashboard, which stops future syncing.
5. Acceptable use. Use the portal only for your own care. Do not upload content that is unlawful or that infringes someone else's rights.
6. Your information. Our handling of your information is described in our Privacy & Consent notice, which forms part of these terms.
7. Changes. We may update these terms as the portal grows. If we make a material change we will note a new version here and, where appropriate, ask for your consent again.
